'Enterprise Logs' paperback; and again in Star Trek Annual #1 in the U.K.
This was the first Star Trek comic ever published.
Comment:

Nitpick: The Gold Key Star Trek comics were of course, rife
with inaccuaracies. Much of the terminology and exposition in this book
seems entirely out of place, considering that the writers and artists of
these tomes had never actually watched Star Trek. This is the beginning
of many issues where Kirk begins by stating which Galaxy the Enterprise
is visiting, Galaxy Alpha. I suppose it could be assumed that 'Galaxy Alpha'
could mean the Alpha Quadrant of our galaxy.
